Services for Families

At the time of admission, the resident and family meet with various members of our team to share information about the resident’s wishes and routines. Together, an individualized plan of care is developed to ensure quality care. After 6 weeks from the admission date, the family is invited to an IDT (interdisciplinary team) conference. Supervisors from each department in the home attend and there is open discussion regarding the ongoing development of each resident’s care plan.

Family Council

Family members are an important part of our team and the home welcomes their input and participation in the resident’s care. Our family council meets quarterly and is made up entirely of the families of our current residents. This allows family members the ability to communicate their concerns or ideas to our management staff and help us to continually improve as a home. All family members are welcome so come join us at any meeting; we love your feedback!
